Name Change - can be completed in your core account at https://core.tn.gov
- Updated price lists must be available at time of inspection.
- Licensed Manager or owner to appear in front of the board.
- $260.00 fee must accompany application ($60.00 database change; $200.00 re-inspection fee).
Establishment Location Address Change - can be completed in your core account at https://core.tn.gov
- Updated price lists must be available at time of inspection.
- If establishment is on septic system, attach letter of approval of septic system from the county health department.
- Letter from zoning authority to verify location is properly zoned for funeral establishment.
- $575.00 fee must accompany application.

Manager Change
The Manager:
Will need to update their new place of employment online at, https://core.tn.gov
The Establishment:
Please submit change via CORE. There is also a $60 fee to change the manager of the funeral / burial establishment.
