Change of Sponsor or Employment

In the event a sponsor is replaced or changed during the course of the apprenticeship, this form must be completed and emailed to the Board at funeral.cemetery.board@tn.gov. Rule 0660-04-.03 requires an Apprentice to notify the Board of a change in the individual sponsoring the apprentice or a change in the establishment employing the apprentice within ten (10) days of the change.
