What do I do if I didn't receive my license in the mail after initial approval or renewal?

You can always request a duplicate license on CORE for a $25 fee.

If the address we have on file for you is the correct address, we ask that you wait six weeks for it to arrive, then you will need to request a duplicate in writing by letter or email giving us your name, license number, address, and stating that the address we have on file is correct. We will send a new license to you at no charge.

If your license is returned to us because the address was not correct, we will mail it back to you at no charge once you notify us in writing of the address change and request.

We do not send a duplicate license out for free it's been six months or more since the original was sent out. You'll have to request a duplicate on CORE for $25 in this case.

What do I do if I'm trying to get my transcripts from a school that has since closed?
